Jahre nachdem Stilwater besetzt wurde haben sich die "Third Street Saints" von einer Straßenbande zu einem wohlklingenden Markennamen entwickelt: Saints Sneakers, Saints Energydrinks und "Johnny Gat bobble head dolls" gibt es nun auch in Geschäften.

Die Saints sind die Könige von Stilwater, aber ihr Berühmtheitsstatus blieb nicht unentdeckt. Das Syndikat, eine legendäre, kriminelle Bruderschaft, die ihre Fänge über die ganze Welt ausgebreitet hat, kehrt ihre Aufmerksamkeit den Saints zu und verlangt Tribut. Doch unerschrocken führen Sie den Widerstand gegen das Syndikat an und erreichen Steelport, eine ehemalige Metropole, die unter der Herrschaft des Syndikats zu einer Stadt der Sünde verkommen ist.

Landen Sie mit einem Panzer aus der Luft, befehligen Sie einen Luftangriff auf eine mexikanische Wrestling-Gang mit Satellitenunterstützung und verteidigen Sie sich selbst, bewaffnet nur mit einem Sex-Spielzeug, gegen gut trainierte Militärtruppen in seltsamsten Umgebungen und entfachen Sie einen Häuserkampf der Steelport in Schutt und Asche setzen wird.
